  • Andres Kohler, PMHNP-BC | Psychiatric Nurse Practitioner | Paperflower Psychiatry

Meet Andres, a board-certified psychiatric nurse practitioner at Paperflower Psychiatry. Andy offers holistic, compassionate care and specializes in religious and cultural trauma. Accepting new virtual patients in Arizona. Andres Kohler Hi! My name is Andres, but you can call me Andy if you’d like. I’m a psychiatric nurse with a wealth of experience across various settings, including inpatient hospitals, addiction medicine, child and adolescent psychiatry, adult psychiatry, crisis units, and geriatric psychiatry. My deepest passions lie in addressing religious and cultural trauma, advocating for LGBTQ+ issues, and supporting individuals with complex conditions like bipolar disorder and schizophrenia. I chose to become a psychiatric nurse practitioner out of frustration with the lack of inclusive, compassionate care often provided by healthcare professionals. Life has taken me on an exciting journey—from my roots in California to Massachusetts, Maine, and now possibly setting my sights on Arizona! I’m also fluent in both English and Spanish. When I’m not working, I love unwinding with a good book, playing with my three dogs, and relaxing in my hot tub. I’m also a huge fan of lavender and tea—small comforts that keep me grounded. Above all, I’m dedicated to creating a safe and inclusive space for my clients, where they can feel seen, heard, and supported in their mental health journey. What is your favorite movie: It’s a tough toss-up between Avatar and Inside Out 2. Avatar has amazing graphics, a great storyline, and so much adventure. I love the idea of Mother Nature fighting back and protecting all her creatures—it’s such a cool concept! On the other hand, Inside Out 2 had me crying one minute and laughing the next. I really connected with the characters, especially anxiety, joy, and sadness. It’s such a powerful kids' movie that’s just as meaningful for adults. LGTBQIA+ Mental Health As a member of the LGBTQ community, Andy is passionate about providing quality care -- especially at this time in history. Religious Trauma Andy understands the severity of how religious trauma can cause guilt, depression and other symptoms over the lifespan. Read more in his Q&A. Bipolar Disorder Andy has a strong ability to evaluate for and treat bipolar disorder -- even the cases where it feels like you've tried everything and it's just not working. Q&A What is your favorite music: My taste in music is all over the place. I love folk/alternative rock, R&B, blues, jazz, country, EDM, reggae, and even cumbias/reggaeton. Some of my favorite artists are Alicia Keys, Maná, Fleetwood Mac, Local Natives, The 1975, Goth Babe, and The Weeknd. Basically, I’ve got a playlist for every mood. Favorite superhero? That’s easy—my mom! She’s the most loving, supportive, and selfless person I know. She immigrated from Mexico in her early 20s, not knowing English, with just a few coins in her pocket and a sweater on her back. Despite all the challenges she faced, including severe respiratory issues and anxiety/depression, she always made sure my brother and I had a home-cooked meal on the table and felt so loved. She taught me to never judge a book by its cover and to always share a smile. I’m beyond proud to be her son and would choose her as my mom in every lifetime. But, okay, I’ll admit—Superman is pretty cool too! If you could have a superpower, what superpower would you pick? Telekinesis, hands down! I mean, think about it—you could lift yourself and others to fly anywhere. No more chores either; you could clean the house just by thinking about it! Plus, you could control the elements if you really wanted to go all-out. It’s the ultimate power. Tell us about your pets: Of course! I have three amazing dogs: • Molly: She’s a 7.5-year-old white Labrador Retriever and the sweetest, most gentle soul you’ll ever meet. Molly is always wagging her tail with a toy in her mouth and is perpetually hungry. She originally belonged to my husband, but after seven years, she’s 100% my dog too. Her nicknames are Mammas and Moo Moo. • Levi: Levi is a 6.5-year-old red Shiba Inu, and honestly, he’s more like a cat than a dog. He’s super smart (sometimes too smart) and loves nothing more than napping. If I lie down, he’s instantly snuggling and snoozing right beside me. Levi was a birthday gift from my husband because I’d always dreamed of having a Shiba. His nicknames are Shiba Shiba, Inu, and Shibs. • Olive: Olive is a 2-year-old red Goldendoodle and the biggest cuddle monster you’ll ever meet. She’s like my shadow—always following me around and needing to be close. Every morning, she jumps onto the bed and lays on my chest before I even get up. She’s obsessed with ice and carrots and was the runt of her litter (and the only red one!). Her nicknames are Beanie, Bean, and Oliebean. If you had 3 wishes, what would you wish for? A world with no pain, suffering, hunger, fear, or hate. Everyone deserves to feel understood and validated. We could all use more love, empathy, and smiles. 2 For all animals to feel loved, safe, and free. 3 To be able to communicate across all languages, cultures, and even species—imagine how much we’d learn from each other! Fun fact about you: I LOVE dogs—like, seriously love them! When I was a kid, I wanted to be a veterinarian just so I could play with dogs all day. My family used to breed Rottweilers, and I’d often go “missing” only to be found cuddling in the whelping pens. Even now, I can’t resist running up to a dog I see and giving them all the pets and hugs. Recently, my friends threw me a surprise birthday party, and the theme was Shiba Inus (my favorite breed). It was honestly the best day ever! What are ways beyond medication that can improve your mental health? Making time for daily self-care is huge! Yoga, massage, aromatherapy, and mindfulness meditation are my go-to recommendations. Getting outside and soaking up some nature is also underrated—it’s one of the best free medicines out there. And sleep! Good sleep is non-negotiable. If you’re not sleeping well, people are more likely to feel anxious, fatigued, and depressed, with poor concentration, paranoia, or even hallucinations. If a patient comes to me feeling off, I always ask about their sleep first. You've mentioned being passionate about working with those with religious or cultural trauma -- can you tell us about that? Growing up as a gay, first-generation Mexican-American in a deeply religious environment was really tough. I’ve experienced firsthand how damaging religious and cultural trauma can be to someone’s mental health. Because of those experiences, I feel so passionate about helping others who’ve been through similar struggles. Everyone deserves to feel loved and accepted—no matter where they come from or what they’ve been through. Are medications lifelong? Does a patient who sees you HAVE to be on medication? Mental health treatment looks different for everyone. I’m a big believer in therapy, but if you want medication, I’ll support you. If you don’t, that’s okay too. I’ll always give my professional opinion and recommendations, but at the end of the day, it’s your choice. Medications can be lifelong for some people, but not for others. If you decide to stop taking medication, I’ll help you do it safely. You don’t have to be on medication to work with me—I also offer therapy as a standalone option. If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? I’m here to support you, not judge you. My goal is to create a safe, comfortable space where you feel heard. Think of an evaluation as more of a conversation—it’s not an interrogation. I’ll check in with you throughout to make sure you’re okay. And if there’s something you’re not ready to talk about, that’s completely fine. My hope is to build trust so that, whenever you’re ready, we can explore those things together. What are your thoughts about treating adult ADHD? Adult ADHD is real, and it doesn’t just affect kids. Adults (and even older adults) can experience ADHD too, sometimes for the first time in life. Living with ADHD often feels like your brain is juggling 10,000 things at once, making it hard to focus. I’ll work with you to find the best treatment plan that works for your unique needs. Do you have any personal mental health experiences you can share with us? Absolutely! I’ve always been an anxious person, even as a kid, and I’ve gone through some significant traumas that led to anxiety, depression, and PTSD. I’m currently on Lexapro for my anxiety, and it’s been such a game-changer. I share my experiences openly because I want to help reduce the stigma around mental health.

BIPOC Mental Health Tatiana is passionate about working with and providing quality care to people of color. Anxiety It is no secret that seeking psychiatric care is anxiety-provoking (when you already have anxiety), but Tatiana is a calming, non-judgmental presence who serves as an expert in anxiety medication. Q&A What is your favorite movie? Avengers: Infinity War (made me happy cry at one point), the Harry Potter Series (read all the books too), anything X-men, of course the Jurassic Park movies, and Finding Dory because I experienced so many emotions when I watched it. I’m also a BIG fan of horror films. The Exorcism of Emily Rose and As Above So Below are probably my faves. Favorite superhero: Hmmm… Does El from Stranger Things count? I’d also say Storm and Spiderman. If you could have a superpower, what superpower would you pick? Either have the ability to travel to other dimensions (because maybe things are better elsewhere) or have the ability to replicate myself and others (like Hermoine did) so we can be in multiple places at once. Favorite bands/singer? I’d say my favorite artists are SZA, Moliy, Amaarae, Tobi Lou, Alina Baraz, Drake, and KYLE. I also LOVE reggaeton. I like listening to Rauw Alejandro, Karol G, Sech, Jhay Cortez, and of course Bad Bunny. If you had 3 wishes, what would you wish for? First and foremost, I’d wish for a world where everyone was safe, happy, food and shelter were a human right/free, everyone was caring and accepting and looked out for one another, and capitalism didn’t exist… 2. To have the means to give my family everything they deserve (especially my mom) ♥️ 3. To see (and potentially pet) all the cute/cool animals in the world. What is your favorite food? I’m Colombian and Salvadoran. Our food is amazing (e.g. empanadas, arepas, tamales, pupusas). Oh wait, I just thought of another. I used to have 2 chickens as pets. We called one of them chicken parm… Tell us about your pets. I have 2 fur babies, Kitty aka Mamow and JJ. Kitty is 13 years old, and my family has had her since she was a teeny tiny kitten that fit in the palm of my hand. Her original name is Kitty, but we used to call her “mi amor” which later merged into “Mamow”. I now mostly call her “Mamow”. JJ (original name Junior Junior) is 6 years old and one of Mamow’s babies. He’s a big softy. But Mamow? Not so much. What are you most passionate about in the mental health field? Recognizing that mental health conditions don’t exist in a vacuum. Trauma, the social and political climate, financial insecurity, medical problems, and a number of other things can all impact mental health. To me it’s so important to always explore and understand the context in which someone is experiencing life. I also strongly believe that various forms of oppression (racism, homophobia, transphobia, sexism, etc.) can contribute to the onset of a new psychiatric condition or exacerbate an existing condition. In my opinion this needs to be recognized and acknowledged more in the field of mental health/psychiatry. What are your thoughts about treating adult ADHD? I think it’s entirely possible that ADHD went undetected and/or untreated in childhood and now is more of an issue for someone as an adult. With further assessment, I’m definitely open to treating adult ADHD. What is your philosophy on using medication to treat mental health? Are medications lifelong? I believe it’s my responsibility to provide as much education about the psychiatric condition(s) and medication(s) as I can for us to then have a conversation about treatment. You know better than anyone else what your experience is and what your goals are. My goal is that we come up with a treatment plan together that is appropriate and doable, whether that includes medications or not. I think medications can be lifelong, but they definitely don’t have to be. It varies person to person. What are other things you feel could be beneficial to improve mental health other than medications? Support. And this can look like a lot of different things. Financial support, therapy, an open ear, skills teaching, school support, childcare, a solid support system, and so many other things. But ultimately, medications unfortunately won’t solve everything. Support is just as important, if not more. Talk to us about your experience working with trauma and dissociative disorders: As a registered nurse, I worked on a unit that provided treatment to patients with extensive trauma histories. Our patients typically presented with depression, PTSD, dissociation, and/or Borderline Personality Disorder. Given the current state of the world, our patients were usually cis-women and folks who identified with the LGBTQIA+ community. Life can sometimes be really hard and really unfair. I get that. My goal was to always do my best to create a welcoming, safe, genuine, and compassionate environment. With this as the foundation, there is a lot we can begin to tackle. Do you have any personal experiences regarding mental health you are opening to sharing? I always thought I was just shy and introverted but turns out I actually have social anxiety (LOL). I’ve also experienced different types of trauma throughout my life and that was really hard… but here I am alive and well, and in a place to meet you. If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? That it’s your decision whether to share or not, and how much or how little. These can be really tough topics and they can bring up a lot of different thoughts and emotions. If you want to hold off on sharing until down the road, that’s cool with me too. Just know that if/when you do decide to share, I’m not judging you for your experience. I’m here to support you.

I am more of TV person, I enjoy the evolving story lines. If I had to pick a few it would have be some classics such as Matilda, The Goonies, Little Rascals, and Fried Green Tomatoes. Favorite superhero? No favs, top three Deadpool, Doctor Strange, and Wolverine! If you could have a superpower, what superpower would you pick? Teleportation! I love to travel and who wouldn’t want to get to their destination in seconds?! Favorite bands/singer? I can listen to almost anything. I love listening to the 90s for a feel-good mood. I also enjoy alternative rock, punk, hip hop, reggae, indie, rap, pop, and country. Top favs across the board would be The Offspring, Tool, Alkaline Trio, Train, Sublime, Garth Brooks, The Distillers, Ice Cube, Dre, Lizzo, and Taylor Swift (no apologies). If you had 3 wishes, what would you wish for? Ensure everyone has a home For my children to live their lives happily and never be in need of anything Level up quickly on any skill I choose What diagnoses are you most comfortable and experienced working with? I am most comfortable treating anxiety, PTSD, mood disorders, and ADHD/neurodevelopmental disorders. What are you most passionate about in the mental health field? I am passionate about education, listening to my patients, and empowering them to advocate for themselves to better overcome the barriers that have existed for so long. Stigma remains a significant obstacle in mental health, preventing many from seeking the care they need. If I can change even one person’s perspective on behavioral health, it brings us one step closer to meaningful change. What are other things you feel could be beneficial to improve mental health other than medications?: There are several studies that recommend therapy as first line treatment in many conditions. In addition, lifestyle changes such as exercise, proper nutrition, and good sleep can help further promote a good mood. I find mindfulness techniques to be very helpful in improving motivation and reducing stress levels when patients are stuck on how to start. What is your philosophy on using medication to treat mental health? Are medications lifelong? Does a patient who sees you HAVE to be on medication? Medications are just one of the tools we have in our behavioral health bag. They are not always needed or wanted. Therapy, exercise, lifestyle changes, and supplements are often incorporated with treatment plans in variable combinations. My experience working with medications is that they can help make day to day functioning and participation in therapy easier if you’re having a hard time. The question comes down to, “How much are you struggling?” Tell us about your experience working in behavioral health the past few years: My experience with behavioral health began as a nurse working in the neurology department and later with patients experiencing crisis in the emergency department. Through my experience as a nurse I was able to fully realize the impact of untreated mental health and the difficulty there was in obtaining that care. This led me to pursue becoming a psychiatric mental health nurse practitioner and over the last couple years have been working primarily in private practice. My experience includes treating patients 6 years old and up, combining holistic and medication management techniques. I also have a special interest in psychedelics and have experience providing Spravato and IV ketamine treatment to patients experiencing treatment resistant depression. Do you have any personal experiences regarding mental health you are open to sharing? I come from a culture where mental health is solved by “pulling up your pants,” or “trying harder.” No matter how high my pants were and how much I kept pushing, it wasn’t enough and it just led to more issues. I finally exhausted myself and realized mental health was for me, not anyone else. Only I could determine what was right for myself and quite frankly it was no one else’s business. How do you feel about marijuana? Marijuana is legal in Arizona and patients have the right to consume if they wish. Marijuana like any other drug or supplement has its risks and benefits. As a provider I acknowledge that patients benefit from different avenues of care. Every BODY is different. Some people do well with moderation and others struggle and exacerbate their symptoms. Together we can explore what is right for you in a judgement free zone. What are your thoughts about treating adult ADHD? There shouldn’t be any difference in the treatment of adult ADHD. That is, every condition should be assessed for and ruled out when meeting new patients. ADHD should always be considered for patients struggling with concentration, self-esteem, depression, and anxiety. Mental health has not been perfect in the past, and many adults including myself were not diagnosed appropriately as a child. Of course stigma plays a huge role in this. I want my patients to feel comfortable asking about any symptoms or concerns they have without fear of being judged or discriminated.

Q&A What is your favorite movie? ‘Children of Men’ – beautiful, hopeful, tragic, & haunting. …But can I also say TV shows? Succession or Handmaid’s Tale …Or books? ‘The Road’ by Cormac McCarthy or ‘The World to Come’ by Dara Horn Favorite superhero? I had a mad crush on Christopher Reeves’ Superman as a kiddo. But favorite now? Black Widow - I love Russian history, ballet, and confident strong women – so, she fits the bill! Or, oh wait, maybe the 1997-Gwen-Stefani-soundtracked-strong-woman-fighter-pilot Captain Marvel. Okay, both. If you could have a superpower, what superpower would you pick? I would love to have the ability to speak any language. But more in the true ‘superpower’ category - probably teleportation – it would REALLY help with school dropoff & pickup and all the activities (and I could travel and use my language super power!). Favorite bands/singer? My 10-year-old daughter is an incredible musician, so I love her – and recently we’ve been listening to a whole lot of things: Jax, Lady Gaga, Adele, Paramore, & Jack White. But all-time, long-time? Eminem. If you had 3 wishes, what would you wish for? For our incarceration system to be completely overhauled – trauma- focused treatment, vocational training, true reintegration. For endless, bottomless patience – with my kids, my spouse, my laundry, I-10 construction… To have a personal chef plan for, shop for, and cook everything all the time What is your favorite food? A good Monte Cristo sandwich What are you most passionate about in the mental health field? High quality & ACCESSIBLE Perinatal Mental Healthcare.. Accurate screenings and appropriate 1) Autism Spectrum Disorder and 2) ADHD diagnoses in girls and adult women What are some experiences you have had that make you unique? -My original career path was education - I was a high school history teacher. I quickly realized that I didn’t really want to do that forever (really kind of knew even in college – but I much preferred reading, writing, and studying abroad to early morning lab class for my pre-med/physical therapist dreams), so after teaching for 5 years and growing up a little bit, pursued nursing school. -I studied abroad in Russia for a semester in college. -I have been a Labor & Delivery Nurse for the last 10 years and LOVE it. What is your philosophy on using medication to treat mental health? Are medications lifelong? Does a patient who sees you HAVE to be on medication? Psychiatric medications can be life-saving scaffolding in the journey to managing your mental health. Sometimes they are a lifelong; sometimes they are a situation- or life-season- specific – but you are completely in control of that long-term plan and it will always be an ongoing discussion. I am a huge advocate of integrating non-medication tools into your toolbox– self-awareness, routine, practical life changes, sunlight, activity, community, and self-compassion (the list goes on) can be small-but-huge steps to transformation. Sometimes medication can be the step to take in order to be able to build in life management skills. As your provider, I will always make sure you are fully educated about medication choices as well as talking you through non-medication management techniques! If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? You are not at fault for what happened to you. And I believe you. You didn’t deserve it – and there are absolutely no exceptions. You. Didn’t. Deserve. It. And. It. Was. Not. Your. Fault. We can talk about it when and if you are ever ready. Or I can help find the perfect people to help you process it if and when you are ready. There is no pressure or judgement. Do you have any personal experiences regarding mental health you are open to sharing? With each of my pregnancies and children, I struggled with varying degrees of depression, anxiety, OCD, and psychosis. The huge gap in care was mind boggling to me – and started my passion to transform the perinatal mental health field. As an adult I was diagnosed with ADHD and (very recently) Autism Spectrum Disorder – making so many things about my life make so much more sense and transformed my ability to manage life. How do you feel about medical marijuana? Pro - I think it can have fantastic benefits for certain things, but definitely will always talk to you about how it is impacting you and your treatment/management. Tell us about your pets: Moby – our all-white, 11-year-old, chilliest cat on the block. Marlow – our new (5 months old in November) Bernedoodle puppy. So fun and funny and naughty as all get out. What are your thoughts about treating adult ADHD? I think it is tragically underdiagnosed! Undiagnosed neurodivergence is a huge risk factor for depression & anxiety. If you weren’t an always-talking-out-of-turn, always-in-trouble, can’t-stay-in-your-seat, six-year-old white boy, you can’t possibly have ADHD. Right? Sigh. What if you were a people-pleasing rule-following girl who did great in school but put EvErYtHiNg off ‘til the last minute and now, as an adult, CaNnOt manage your laundry or kid-pick up schedules or ever quiet your brain enough to nap? Or you found that drugs or alcohol were the only way to quiet your brain or manage your anxiety? There are so many factors at play with mental health - I will always consider ADHD or ASD diagnostic criteria during an initial evaluation, and will make sure you feel clear on the risks and benefits all of the options – medication and non-medication. Tell us one fun fact about yourself: As a 6th grader, I wrote a musical about the History of Ancient Egypt. I was also the sole cast and audience member.
